Scope
This policy is provided under the Washington My Health My Data Act (RCW 19.373). It covers "consumer health data" collected through this website, meaning information that identifies you and relates to your physical or mental health, including the fact that you are seeking health care services.
It does not cover protected health information governed by HIPAA. Once you become a client, your clinical records are covered by Mindful Therapy Group's Notice of Privacy Practices instead.
Consumer Health Data Collected and Why
- Contact form information: your name, email address, the type of service you are asking about (individual, couples, or family therapy), and anything you write in your message, which may describe why you are seeking therapy. This is used only to respond to your inquiry and help you get connected with care.
- Technical information: when you visit this website, the hosting provider automatically processes information such as your IP address and browser type to deliver the site and protect it from abuse. This information is not used to identify you or to infer anything about your health.
Consumer health data collected through this website is never sold and is never used for advertising.
Sources of Consumer Health Data
- Directly from you, when you submit the contact form or email me.
- Automatically from your device, when you visit this website.
Consumer Health Data Shared
Contact form information passes through the service providers listed below so that it can be delivered to me. If you ask to schedule an appointment, your name and contact information may be shared with Mindful Therapy Group so their intake team can book it. No other consumer health data is shared.
Third Parties and Affiliates
- Formspree, the form processing service that delivers contact form submissions
- Cloudflare, the website hosting provider
- My business email provider, which receives and stores contact form messages
- Mindful Therapy Group, for scheduling and intake, only when you ask to schedule
Jackson Michener Psychotherapy, PLLC has no affiliates.
Your Rights
Under the Washington My Health My Data Act, you have the right to:
- Confirm whether your consumer health data is being collected or shared, and access that data, including a list of all third parties and affiliates it has been shared with
- Withdraw your consent to the collection and sharing of your consumer health data
- Have your consumer health data deleted
To make a request, email jackson@jacksonmichener.com with the subject line "Consumer Health Data Request." I may need to verify your identity before acting on it. You will receive a response within 45 days. If more time is reasonably necessary, the deadline may be extended once by an additional 45 days, and you will be told why. Requests are free of charge.
The right to deletion applies to information collected through this website. It does not apply to clinical records, which must be kept as required by law.
Appeals
If your request is declined, you may appeal by replying to that decision with the subject line "Appeal." You will receive a written decision, with an explanation, within 45 days. If your appeal is denied, you may submit a complaint to the Washington State Attorney General at atg.wa.gov/file-complaint or by calling 1-800-551-4636.
